RxMedChart® Medication Charts

A complete medication chart legally recognised as a valid PBS prescription – minimising risk & error in aged care.

Australia’s leading medication chart as a prescription.

The RxMedChart system is Webstercare’s computer-generated version of the National Residential Medication Chart (NRMC), representing a ‘single source of truth’ when it comes to a resident’s complete and current medication profile.

A resident’s entire medication profile is printed onto the chart in a simple, consistent and clear layout – improving the quality of information available to all members of the health care team. Designed to reduce risk and increase efficiency for the facility, pharmacy and doctor, medical information is printed by the pharmacy and then reviewed and verified by the prescriber. This minimizes the risk of error from misinterpreted handwriting.

Benefits

Legal chart as a script*– and supports PBS claims

Complies with the Commonwealth National Health (Residential Medication Chart) Determination 2012. PBS claims are submitted electronically via dispense software.

Prevents medication errors

A resident’s complete and latest medication profile printed in one place. The entire healthcare team refers to this one document which eliminates the duplication of records, time spent in checking and aligning different sources of information.

Eliminates owing scripts

The RxMedChart™ acts as a valid script for up to four months* – eliminating chasing scripts. By reducing the constant script reminders this helps to improve relationships between pharmacies, doctors and nurses.

Dispensing time reduced

Medication profiles are accurate on each prescriber order sheet. This reduces the time spent going through prescriptions, searching for the oldest script and attaching repeats once dispensed.

Ensures continuity of supply for up to 4 months

Most medicines recorded on the RxMedChart™ can be continually supplied at PBS quantities for up to four months – ensuring residents’ continuity of care.

*A separate script must be supplied for S8 medications, prior approval authority prescriptions, S100 and PBS extemporaneous medications.

FAQs

What will I need to set up the RxMedChart system?

  • Webstercare’s Medication Management Software on Professional plus
  • Internet access
  • A colour printer

What training is provided during an implementation?

  • A 1 hour Pharmacy Data entry training session (via LogmeIn)
  • An online training module is available for the pharmacy and RACF (residential aged care facility)
  • GoTo meeting training’s which are remote and live training sessions can be organised for the RACF

How long will it take to setup

Generally we recommend a 4-6 week lead time to allow enough time to conduct the training and complete the data preparation. However, it can be tailored to the customer’s needs. It is dependant on a variety of factors i.e.: # of residents, # of servicing pharmacies and # of doctors.

What our Customers Say

  • Kathryn Mussing, Residential Manager

    Baptist Care Group

Medication safety has improved enormously. We have gone from an unacceptable number – both packing and administration errors – to virtually zero now.
  • Kathryn Mussing, Residential Manager

    Baptist Care Group

Staff had difficulty reading the doctors’ writing with the previous system. They had difficulty flipping over several pages to find the nonpacked medications, as these weren’t identified like they are on the RxMedChart. The stop dates weren’t clear and the doctors hated having to rewrite all of their medication orders every four months.
